Te Whatu Ora Vision Testing

On Monday 24th July Hauora a Toi Bay of Plenty, Te Whatu Ora Vision Technicians will be visiting your
child’s school to carry out routine distance vision screening.All students in year seven are included as part of the National Screening Programme.
If your child is absent on this day (or any catch up date) they will be tested the following year, as a year
eight student.
The screening test used is for distance vision only. It is not a full eye examination. If you have concerns
about your child’s close vision, please book an appointment with an Optometrist.
The test is conducted by the student covering one eye and reading letters from a chart at a distance of four
meters. We will only notify you (by post) if the screening results require you to take your child to an
Optometrist for further testing.
If you Do Not wish to have your child’s vision tested, please sign the form below and return it to school
before the testing date.
